Driver Improvement School Form [PDF] Download this form if you have been authorized by a judge or a department of motor vehicle official. Complete the form and submit it to the court clerk's office along with written authorization. Payment of $50.00 is required. Parking Ticket Appeal Form [PDF] This form is for parking tickets only. The appeal must be filed with the clerk's office no later than three days of the ticket issue date. Frivolous appeals will result in the attachment of court costs of $66.25. Once this appeal is entered by the clerk, you must notify the clerk to schedule your court date. Although not required, hiring legal counsel in recommended.
